Insurance Information

Milford-Franklin Eye Center accepts a wide variety of major health insurance plans. Please call your insurance company to verify your specific plan’s coverage. Not all plan types are accepted. It is advised that you call your insurance company before scheduling your visit to ensure your visit will be covered. Be sure to bring a photo ID and your insurance cards with you at the time of your appointment.

Billing Information

Full payment is required at the time of your appointment. We accept various methods of payment, including cash, check and cards (including Visa, MasterCard, Discover and American Express.)

POS/PPO Plans:

Most members covered under a POS or PPO plan are required to make some form of payment for services rendered to them which may be a copayment, coinsurance or deductible. If your plan requires a copayment, we must collect it at the beginning of your appointment before you see the physician. We are required by contract with your insurance company to collect copays, deductibles and coinsurances.

HMO Plans:

Most people covered under HMO plans will owe copayments as well. These will be collected at the beginning of your appointment before seeing the physician. We are under contract with your insurance provider and are required to collect these payments.

Balances on Account:

Previous balances are to be paid in full prior to additional services being rendered. If necessary, a payment plan can be established at the discretion of the practice manager.

Collections:

After several statements have been issued, we will utilize an outside collections agency to obtain balances that are owed, you will be responsible for any agency or attorney fees.

Obtaining Insurance Referrals:

Being referred by your doctor is different from an insurance referral. Many Masshealth plans, POS plans and HMO plans require referrals to cover your visit with us as we are a specialist office. We request referrals as a courtesy however, it is ultimately the patient’s responsibility to obtain an insurance referral from your primary care physician prior to your visit. Claims denied due to a lack of referral will be billed to the patient per your insurance regulations.

Routine Eye Exams:

The insurance billing of your appointment will be based on the diagnosis determined by the doctor at the end of the exam. Although some insurance carriers cover a routine eye examination, the definition of a routine visit does not include a problem-visit during which you complain about a symptom or a medical concern.
